A.C.H.S.  VOLUNTEER FORM

BECOME A CAMARILLO HIGH V.I.P.

(Very Important Parent)

Dear Parents,

We invite you to become a part of our “PTSA Team” by volunteering for those committees that interest you.  Please take a moment to place a check by the committees you’re interested in joining.   You’re not being asked to head a committee, just to help. Getting involved with the PTSA is an opportunity to feel the pulse of your school and make a positive impact on your student’s education.  Everyone is needed and everyone is welcome.  Please have your student return this form to the school office, or mail to Adolfo Camarillo High School, 4660 Mission Oaks Blvd., Camarillo, CA. 93012.

Thank you in advance for all the help and cooperation that you, the parents, show and give throughout the year.  Please feel free to call me at any time with your questions or comments,

Rochelle Sullivan, PTSA President, 987-2418
